In-Home Support Services
Support Service Organisations
In-home support services are provided by 3 alternate organisations. To identify who the provider is in your local area simply check the area and postcodes below.
Alliance Partnership
Council is the facilitator of the Alliance with Hesse Rural Health, Great Ocean Road Health and Mecwacare.
The Alliance works together to ensure the needs of the Surf Coast Shire residents are being met.
Available services with new providers – same as before
- Domestic assistance
Assistance with household jobs including laundry, grocery shopping, meal preparation and vacuuming cleaning
- Personal care
Support with bathing, grooming, dressing, fitting of aids, daily living activities, shopping, meal times, and help to get to medical appointments.
- Respite Care
Regular or occasional respite services, giving primary carers a break.
- Delivered meals
Meals delivered to your door.
- Property and home maintenance
Support with small jobs including changing light bulbs, limited gardening assistance, minor repairs and small modifications to the home.
Who is eligible?
Commonwealth Home Support Program
Surf Coast Shire residents aged 65 or over (50 years and over for Aboriginal and Torres Strait Islander people) who experience difficulty carrying out the tasks of daily living or need support to connect socially in their local community.

Step 2: Have a face-to-face assessment

My Aged Care may arrange for a trained assessor to come to your home. With your consent they will assess your care needs and eligibility for services. They will then work with you to develop a support plan which addresses your needs, goals and preferences. Someone else can be with you during this visit.
Step 3: Plan for my services and find out about costs
How much does it cost?
The cost will depend on your or your family member’s needs and source of income. A final price will be determined at the time of assessment, with eligibility for either the Commonwealth Home Support Program (CHSP). Costs are subsidised. Your actual fee will be determined at the time of assessment.
Home and Community Care Program for Younger People (HACC PYP)
Individuals who are under the age of 65 years (under the age of 50 years for Aboriginal and Torres Strait Islanders) and who experience difficulty carrying out the tasks of daily living are eligible to apply for HACC PYP supports. Services are targeted to younger people with moderate, severe or profound disabilities and their unpaid carers.
Bellarine Community Health is the HACC PYP service provider for Surf Coast Shire.
Contact Bellarine Community Health on 1800 007 224 to discuss eligibility, or you can be referred to Bellarine Community Health HACC PYP by your GP or community health organisation.
